Siquijor is known across the Philippines for its folk healers, locally called mananambal, who blend prayer, herbal medicine and ritual — a tradition that predates tourism and is still practised today, mostly for locals, with visitors welcomed to observe or take part.

The healer in San Antonio uses two techniques: wrapping and smoking. In the wrapping ritual, the patient is wrapped in a large white sheet while the healer recites prayers, said to form a symbolic barrier against negative energy. In the smoking ritual, sacred earth is heated in a pot placed beneath the patient's seat, releasing a gentle smoke believed to cleanse and rebalance. The healer may follow up with herbal remedies made from local plants.
